So why do some say it harms the mouth? The answer is a mix of misunderstanding and bad headlines.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Human Microbiome Project of 2007 illustrated the mouth is home to hundreds of previously unrecognized <\/span><i><span style=\"font-weight: 400;\">good<\/span><\/i><span style=\"font-weight: 400;\"> bacteria. Dentists used to call plaque \u201c<\/span><i><span style=\"font-weight: 400;\">biofilm,<\/span><\/i><span style=\"font-weight: 400;\">\u201d but plaque is a biofilm <\/span><i><span style=\"font-weight: 400;\">infection<\/span><\/i><span style=\"font-weight: 400;\">, and healthy biofilm is protective and the <\/span><i><span style=\"font-weight: 400;\">goal<\/span><\/i><span style=\"font-weight: 400;\"> of good oral care. Studies show 40% of adult biofilm develops in childhood, which illustrates the tenacity of biofilm and challenges any idea \u201cmouthrinse\u201d could damage it. So, how did this wrong information become news?<\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 <\/span><b>Specificity:<\/b><span style=\"font-weight: 400;\"> Any liquid \u201cwashes\u201d the mouth, so if we discuss \u201cmouthwash\u201d we <\/span><i><span style=\"font-weight: 400;\">must<\/span><\/i><span style=\"font-weight: 400;\"> be s<\/span><i><span style=\"font-weight: 400;\">pecific<\/span><\/i><span style=\"font-weight: 400;\">:<\/span><\/li>\n<\/ol>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 \u00a0 <\/span><span style=\"font-weight: 400;\">Chlorhexidine is a hospital-grade <\/span><b>antiseptic<\/b><\/li>\n<li><span style=\"font-weight: 400;\"> \u00a0 <\/span> <span style=\"font-weight: 400;\">Listerine is an <\/span><b>essential oil<\/b><span style=\"font-weight: 400;\"> (EO) rinse (often wrongly confused with chlorhexidine)<\/span><\/li>\n<li><span style=\"font-weight: 400;\"> \u00a0 <\/span> <span style=\"font-weight: 400;\">ACT is <\/span><b>dilute (0.05%) sodium fluoride<\/b><\/li>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 \u00a0 <\/span><span style=\"font-weight: 400;\">Scope contains <\/span><span style=\"font-weight: 400;\">Cetylpyridinium Chloride (CPC)<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ol start=\"2\">\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 <\/span><b>Misunderstanding:<\/b><\/li>\n<\/ol>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 \u00a0 <\/span><span style=\"font-weight: 400;\">Studies claim blood pressure is affected by \u201cmouthwash,\u201d yet our mouths are washed <\/span><i><span style=\"font-weight: 400;\">as any liquid enters our mouth, <\/span><\/i><span style=\"font-weight: 400;\">mixes with saliva and collects twenty percent (20%) of the mouth\u2019s (planktonic) bacteria. Some liquids are swallowed (as a drink) others expectorated.<\/span><\/li>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 \u00a0 <\/span><span style=\"font-weight: 400;\">Mouth \u201cwashing\u201d occurs at every meal, when we clean our teeth, or have a drink &#8211; even water. The loss of planktonic bacteria is quickly reversed, since eighty percent (80%) of bacteria are safely housed in resilient oral biofilm <\/span><i><span style=\"font-weight: 400;\">that does not get washed away.<\/span><\/i><\/li>\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 \u00a0 <\/span><span style=\"font-weight: 400;\">It takes<\/span> <span style=\"font-weight: 400;\">60-90 minutes to re-establish bacterial levels, which means that any bacterial or nitric oxide testing should occur 60-90 minutes <\/span><i><span style=\"font-weight: 400;\">after<\/span><\/i><span style=\"font-weight: 400;\"> rinsing, drinking, eating, or even a cup of water.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ol start=\"3\">\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 <\/span><b>Nitric Oxide:<\/b><span style=\"font-weight: 400;\"> Sickly, older adults do not generate nitric oxide throughout their body as young people, so the production of nitric oxide in the mouth may be more important with age. Surface tongue bacteria are involved in this, and they <\/span><i><span style=\"font-weight: 400;\">can<\/span><\/i><span style=\"font-weight: 400;\"> be depleted in vulnerable individuals by <\/span><i><span style=\"font-weight: 400;\">Chlorhexidine<\/span><\/i><span style=\"font-weight: 400;\">. Depletion of nitric oxide can create negative systemic consequences for the heart, breathing, blood pressure, and brain health.\u00a0<\/span><\/li>\n<\/ol>\n<ol start=\"4\">\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 <\/span><b>Chlorhexidine<\/b><span style=\"font-weight: 400;\"> is a strong antiseptic which eliminates surface bacteria from biofilm. This is why chlorhexidine is used to disinfect hospital intubation tubes and kill surface mouth bacteria\u2013including on the tongue (where nitric oxide is generated). This explains why <\/span><b>Chlorhexidine<\/b><span style=\"font-weight: 400;\"> can raise blood pressure and make breathing more difficult, especially in sick and elderly individuals. When these studies were reported, media headlines took the word \u201cantiseptic\u201d and used an image of Listerine. This is how the myth about Listerine was wrongly generated.<\/span><\/li>\n<\/ol>\n<ol start=\"5\">\n<li><span style=\"font-weight: 400;\"> \u00a0 \u00a0 <\/span><b>Listerine\u2019s <\/b><span style=\"font-weight: 400;\">active ingredients are essential oils, with mild effects on planktonic bacteria and eucalyptus to stimulate gum growth. (These oils are dissolved in less than 25% ethanol, and it requires 40-80% ethanol to affect bacterial function). Listerine targets Streptococcus mutans that \u201c<\/span><i><span style=\"font-weight: 400;\">cause<\/span><\/i><span style=\"font-weight: 400;\">\u201d plaque problems.
